Program Overview

Understanding Support at Home

Support at Home is Australia's reformed aged care program, effective from 1 November 2025. It replaced the Home Care Packages Program and Short-Term Restorative Care Programme.

The program provides government-funded, coordinated care and services to help older Australians remain living independently at home. It's designed to be more flexible, person-centred, and responsive to individual needs.

The Commonwealth Home Support Program (CHSP) will transition to Support at Home no earlier than 1 July 2027.

Key Changes

  • Single Assessment System for faster eligibility determination
  • Flexible funding based on individual care needs
  • Greater choice in service providers
  • Person-centred care planning
  • Short-term support pathways for recovery and rehabilitation
